OSTEOPATHY AND CLAIMS
We accept a number of third party claims.
These include, Medicare chronic disease management plans (CDM), DVA and TAC referrals.
We also accept private health insurance claims (please see below)
Do you need a referral to see an osteopath? In which cases would a referral be required?
No ! Osteopaths are primary health care practitioners and do not require GP referral UNLESS the patient is under medicare chronic disease management (DCM/EPC) plan and department of veteran affairs. Rebates are available from most private health insurance funds with no referral.
What claims do osteopaths accept?
At Revitalize Osteopathy we accept Medicare CMD, DVA and TAC . We also accept private health insurance claims.
Do you offer concession rates?
Concession rates may apply for pensioners, students (18 years +) or those with a current health care card.
Do you process health funds?
We accept and process private health claims onsite.

Medicare CDM plan:
Is
a government initiative for all Australians. If your pain is chronic, you can talk to your GP and be provided with CDM plan to see an osteopath 5 times per calendar year with a subsidised cost to your consultation.
Step 1: Talk to your GP/family doctor about your long-term pain that requires osteopathic treatment. Your doctor then uses the CDM form to refer you to our osteopath. Keep this CDM referral form with you.
Step 2: Ring our clinic or book online to schedule your appointment.
Step 3: When you have the appointment, bring the CDM form and Medicare card with you. We will process the claim onsite. However, if for some reason you get rejected, then you are responsible to pay for your treatment fees and later claim it manually at your local medicare office.
Transport Accident Commission (TAC):
TAC is a government authority to look after people on our roads.
Step 1: If you have a motor vehicle accident, please note down the date of the accidents and get a TAC claim number.
Step 2: If it was a major accident and you were admitted to hospital then just simply bring your TAC claim number to our clinic. We will use your claim number to check with the authority
DVA (looking after our veterans):
For Gold card holder:
Step 1: Ask your GP (family doctor) to refer you to us through DVA system.
Step 2: Bring your Gold card and referral form to our clinic.
Step 3: We will bulk bill Medicare DVA directly.
For white card holder:
Step 1: Ask your GP (family doctor) to refer you to us through DVA system.
Step 2: Bring your White card and referral form to our clinic.
Step 3: We will need to contact DVA to confirm your eligibility for the treatment.
Step 4: If DVA accepts the treatment claim, we will bulk bill Medicare DVA directly.
